Information and Precautions

We know these are challenging times for you and your loved ones. Like you, our utmost concern is for the health and well-being of each of our patients and to ensure our staff is healthy and able to continue to care for patients. With that being said, we continue to take all necessary cautionary measures and are strictly adhering to and implementing all CDC recommendations and guidelines. We continue to also work with local and state agencies as we fight Covid19. During this time we also recognize how important it is to keep the lines of communication open to give you peace of mind while we continue to meet the care needs of your loved ones while at our facility. As such, the facility will be providing updates of new developments as they take place on this page to keep you updated.

2021 COVID Updates

December 20, 2021

Dear Responsible Parties:

Yesterday (12/19/2021), we learned that 3 new residents had tested positive for COVID-19. The resident were moved into a dedicated COVID (isolation) unit where they will receive care from staff dedicated to that unit. The residents will remain in isolation until they meet the criteria to discontinue isolation precautions. The rest of our patients are being closely monitored. We are also keeping in close contact with Public Health and following all recommendations.


December 17, 2021

Yesterday (12/16/2021), we learned that 3 residents had tested positive for COVID-19. The residents were moved into a dedicated COVID (isolation) unit where they will receive care from staff dedicated to that unit. The rest of our patients are being closely monitored. We are also keeping in close contact with Public Health and following all recommendations.

September 20, 2021

Today, we learned that 1 resident and 2 staff members have tested positive for COVID-19. The staff members are at home (under quarantine) and doing well. The resident was moved into a dedicated COVID (isolation) unit where he/she will receive care from staff dedicated to that unit. The rest of our patients are being closely monitored. We are also keeping in close contact with Madera County Public Health and CDPH to mitigate the spread of transmission within the facility.


September 13, 2021

Please allow this communication to serve as notification that 8 residents and 2 staff members have tested positive for COVID-19. As such, they are being closely monitored. We are also keeping in close contact with Public Health with this new development.

At present, those residents that are positive are being kept in isolation. All employees that are positive and/or exhibiting symptoms are being quarantined in their homes.

We will keep you updated as to this new development. Please be assured that during this time we will keep the lines of communication open to give you peace of mind while we work to ensure the safety of your loved ones and our staff.

Please allow this email to serve as an update as to new developments at our facility relative to COVID-19. We have learned that 2 resident(s) have tested positive for COVID-19. As such, we are continuing to work with Public Health and various local agencies to stop the spread of COVID-19.

Currently, we are continuing to implement all the guidelines of the Centers for Disease Control. Should additional safeguards be added, we will provide you with an update.

May 1, 2021

Dear Families and Staff, yesterday we learned that two newly admitted residents had tested positive for COVID-19. Both residents are now in a designated COVID Zone where they will complete their quarantine under the care of dedicated staff. Both residents are currently doing well, and we expect a quick recovery. We have been in communication with the local health Department, and we are working closely with them and implementing all the guidelines from the Centers for Disease Control (CDC). Our goal is to keep all line of communication open during this time while we are ensuring the safety of your loved ones and our employees.

December 16, 2020

Please allow this email to serve as a notification that on 12/16/2020 we learned that 1 new employee had tested positive for COVID-19. The employee is now at home under quarantine and doing well. We continue to work with Public Health and various other local agencies to stop the spread of COVID-19.

November 3, 2020

Yesterday, we were notified that 3 staff members and 1 resident had tested positive for Covid-19. The 3 staff members are now at home under quarantine. The 1 new resident is now in isolation under the care of dedicated staff members. We continue to work closely with Madera County Public Health and California Department of Public Health on ways to mitigate the spread of Covid-19 within our facility. We will continue to provide you with updates as information becomes available.

October 22, 2020

Responsible Parties:

On 10/21/2020, We learned that 2 resident(s) had tested positive for COVID-19. Both residents have been moved to a designated area of the facility and are under isolation precautions. We have notified Madera County Public Health and California Department of Public Health of the new developments. As such, we are working with Public Health and various agencies to stop the spread of COVID-19.

August 26, 2020 Today, we learned that 3 new residents have tested positive for COVID-19. The three residents have been placed in isolation in a designated area of our facility. We are continuing to work with Public Health and various local agencies to stop the spread of COVID-19 in our facility and community. At this time, we are continuing to implement all the guidelines of the Centers for Disease Control. We also continue to test our residents and staff on a weekly basis per the recommendations of CDPH. Should additional safeguards be added, we will provide you with an update. August 19, 2020

Today, we learned that 2 employee(s) and 1 resident has tested positive for COVID-19. Both employees are now at home under quarantine. The one resident has been placed in isolation. We are continuing to work with Public Health and various local agencies to stop the spread of COVID-19. At this time, we are continuing to implement all the guidelines of the Centers for Disease Control. We also continue to test our residents and staff on a weekly basis per the recommendations of CDPH. Should additional safeguards be added, we will provide you with an update. August 12, 2020

On 8/11/2020, we did learn that one new resident had tested positive for COVID-19. This resident is currently in isolation being taken care of by dedicated staff members. We continue to work closely with our local county public health office. They have recommended for us to continue to test all of our residents and staff weekly. We also continue to work with CDPH on mitigating efforts. We will continue to keep you updated with any new information as it becomes available.


August 6, 2020

On 8/5/2020, we learned that 1 new resident and 1 new staff member had tested positive for COVID-19. The new resident has since been placed in isolation and is currently doing well. The new staff member is at home under quarantine. We continue to follow the recommendation of our local public health to test all our residents and staff on a weekly basis. We continue to work with our local public health and CDPH to mitigate the spread of transmission within our facility. We will continue to keep you updated as information becomes available.

July 31, 2020

Today, we have no new resident or staff cases to report. We are continuing to work with local health officials and CDPH in order to mitigate the spread of the virus within our facility and community. We are testing all of our residents and staff according to guidance provided by our local public health and CDPH. We are working collaboratively with these partners to ensure the health and safety of each of our residents and staff members. We will continue to keep you updated as information becomes available.


July 27, 2020 On July 26, 2020, we learned that 3 residents had tested positive for COVID-19. Today, we also learned that 4 new residents had tested positive for COVID-19. Each of the residents is currently in isolation. On July 26, 2020, and July 27, 2020, we also learned that 2 new staff members tested positive for COVID-19. Both employees are currently at home under quarantine. Today, we also had 6 residents who met the criteria to discontinue isolation precautions. We will continue to test our staff and residents weekly. We also continue to work with CDPH and our local health department to mitigate the transmission of the virus.


July 20, 2020

On 7/19/2020 and 7/20/2020, we learned that 3 more of our residents tested positive for COVID-19. The 3 residents are currently in isolation. On 7/20/2020 we learned that 1 of our staff members also tested positive. The 1 employee is at home and doing well. We continue to work collaboratively with CDPH and Madera County Public Health. We continue to test all residents and staff weekly. We will also continue to keep you updated as information becomes available.


July 18, 2020

On 7/17/2020, we learned that 6 more of our residents tested positive for COVID-19. The 6 residents are currently in isolation. We also learned that 1 of our staff members also tested positive. The 1 employee is at home and doing well. We continue to work collaboratively with CDPH and Madera County Public Health. We continue to test all residents and staff weekly. We will also continue to keep you updated as information becomes available.


July 16, 2020 and July 17, 2020 We were notified that 2 more of our residents tested positive for COVID-19. We have also learned that 3 more of our staff members have tested positive. The two new residents are currently being kept in isolation and being closely monitored. We are in contact with Madera County Public Health and CDPH and keeping them informed of any new developments. All infection control and safety precautions are in place. The facility is conducting weekly testing of both residents and staff. The priority is that of the residents and staff.


July 15, 2020

On 7/14/2020 we were notified that 6 more of our residents tested positive for COVID-19. Those residents are currently being kept in isolation. As such, they are being closely monitored. We are also keeping in close contact with Public Health with any new developments. The facility is working closely with public health at this current time. All infection control and safety precautions are in place. The facility is conducting weekly testing of both residents and staff. The priority is that of the residents and staff. Please be assured that during this time we will keep the lines of communication open to give you peace of mind while we work to ensure the safety of your loved ones and our staff.


July 13, 2020

On 7/12/2020 and 7/13/2020 We learned that 8 new residents tested positive for Covid-19. This brings our facility total to 23 positive residents. We are sad to report that 1 of the residents that tested positive has passed away. We have also been notified that 4 new staff members have tested positive. This brings our current staff total to 15. The 15 staff members are all currently at home doing well and under quarantine. We continue to work with Madera County Public Health and CDPH. All infection control and safety precautions are in place. The facility is conducting weekly testing of both residents and staff. The priority is that of the residents and staff.


July 11, 2020 Yesterday and today we have learned that we have 3 new residents who have tested positive for Covid-19. The residents are being cared for by dedicated staff in our designated area. We also learned that 2 more staff members have tested positive. Both employees are currently at home under quarantine. We continue to work with our local health department and CDPH to mitigate the risk of transmission. We will also continue to test our residents and staff weekly. We will make sure to provide frequent updates as new information becomes available.


July 9, 2020 We have been informed that 9 of our residents have tested positive for Covid-19. All 9 of these residents are currently in isolation. This brings our resident total to 13 positive residents. Thus far, all of these residents are doing well. We have also been informed that 2 more staff members have tested positive bringing our total to 7 positive staff members. We continue to work with the local health department and CDPH on mitigating efforts. We will continue to update you as more information becomes available.


July 6, 2020 We were informed that 3 of our residents have tested positive for Covid-19. Each of those residents that have tested positive is being kept in isolation and all three are currently doing well. On 7/7/2020, we also learned that 1 more of our employees tested positive. The employee last worked on 6/25/2020. The employee is currently at home and doing well. These new cases bring our total to 5 residents and 4 staff. We continue working with Madera County Public Health and CDPH to mitigate the spread in our facility and the community. We will keep you updated as to this new development. July 4, 2020

On 7/3/2020, we were notified that one more of our residents had tested positive for COVID-19. The resident has since been placed in isolation and is doing well. This brings the total of positive residents to two. We are continuing to follow the guidance from the local health department and CDPH. We will continue to provide updates as they become available.


July 3, 2020

We recently learned that 1 of our residents has tested positive for COVID-19. The resident is currently in isolation and doing well. We are implementing all the guidelines of the Centers for Disease Control. Should additional safeguards be added, we will provide you with an update. July 2, 2020

On 7/1/2020 we were informed that 1 of our employees had tested positive for COVID-19. The employee is currently at home under quarantine. The employee last worked at the facility on 6/22/2020. This brings the total number of employees who have tested positive to four. The first being on 5/28/2020, the second on 6/24/2020, the third on 6/25/2020, and the fourth on 7/1/2020. All of these employees are at home and doing well. Thus far, no residents have tested positive for Covid-19. We are continuing to work with Madera County Public Health and CDPH in order to ensure the safety and well-being of all of our residents and staff.

June 26, 2020

Responsible Parties:

On 6/25/2020 we were informed by Madera County Public Health that one more of our employees has tested positive for Covid-19. This brings our current total to three positive employees. The first being 5/28/2020, the second on 6/24/2020, and the third on 6/25/2020. All three employees are being quarantined in their homes.

On 6/26/2020 we received the remaining results of our second round of testing for residents and thus far, all residents have tested negative.

We are working closely with the local health department and they have advised us to continue to test all of our staff and residents weekly.

June 25, 2020

On 6/24/2020, we were informed by Madera County Public Health that one of our employees tested positive for Covid19. Thus far, we have had two employees test positive; with the previous employee testing positive on 5/28/2020. Both employees are currently quarantining at home and both are doing well. We have been in contact with Public Health since learning of the positive test. The local public health has told us to continue to monitor all of our residents and staff. Public health has also advised for us to continue the testing of all our residents and staff. We will provide more updates as information becomes available.


June 19, 2020 As of today 6/19/2020, we have completed the second round of testing for all of our residents and most of our staff. Thus far, the results of the second round of testing have all been negative. The 1 employee that tested positive on 5/28/2020 remains quarantined at home and is doing well. We will continue to work collaboratively with Madera County Public Health and the Department of Public Health to ensure the well being and safety of all of our residents and employees.


June 12, 2020

To date, mass testing has been completed by all residents and staff. Based on the results of the facility-wide testing only 1 employee has tested positive. This employee remains quarantined at home and is doing well. Meanwhile, the facility received inconclusive results for 1 resident. Currently, the results for this resident are being further investigated/analyzed by public health. The resident under further investigation is currently in droplet isolation. The facility is currently in the process of conducting a second round of testing of all residents and staff. We continue to work collaboratively with Public Health and the Department of Public Health to ensure the well being and safety of all its residents and employees.


June 4, 2020 To date, mass testing has been completed of all residents and staff. Based on the results of the facility-wide testing only 1 employee has tested positive. Meanwhile, the facility has received inconclusive results for 1 resident and 1 employee. Currently, their results are being further investigated/re-analyzed. While the result of the employee’s test is being re-analyzed, the employee remains quarantined at home as well as the employee who tested positive. The resident is currently in droplet isolation. The facility continues to work collaboratively with Public Health and the Department of Public Health to ensure the well being and safety of all its residents and employees.

May 30, 2020

We just learned that one more of our employees has tested positive for Covid19. Currently, the employee is home and under quarantine.

The health of our residents and staff is a top priority. We are working with public health to set up appropriate protocols for residents and staff that may have come into contact with this employee. Rest assured we are following all the recommendations of public health to ensure the health, safety, and well-being of our residents, staff, and community.
